Household Outreach

Our nurse-led service provides a comprehensive household assessment to all our families.​

A visit from our registered nurses will provide an opportunity to assist us to understand the health issues or challenges you are experiencing in your family/household.​

​

This information will help us to work with you and your family to develop a solution focused family plan. The purpose of the plan is to help prioritise the key goals and targets you would like to achieve as a family. Our focus is to support families to implement the plan with regular reviews and assessments to ensure you achieve positive outcomes.

If you are from either an organisation that wishes to refer a family, an individual, for a household assessment by our staff; or if you are wishing to make a self-referral, you can do so by completing out our referral form found here
